Have you considered that they might be decorative piercings? Maybe a nipple piercing and a combination nose/eyebrow ring?

I don't think you should worry. But maybe it is a good Idea to ask him what he thinks he was doing, and why. At least to understand if he meant it as a torture or simply was trying to see if those toothpicks can pierce the teddy bear, or if they will stick or fall off...
hehe- yes the first time i asked him he said he was giving him "spikes" like the ones that porcupine has as a "protection"..
the second time he just said "no reason, just felt like it" and then mentioned how the toothpick went all the way through.
so maybe a science experiment ?
Laila, he's 3 years old, and he's male. He's going to be doing lots of things that neither one of you can explain until- well, Judgment Day. :D
Noor, insha'Allah, will make a bit more sense- being a girl, and all.
